This is a Triplexer MX-3000DM with 2 M-type intermediate connectors.
Operation has been confirmed.

Frequency range is 1.6-160MHz, 350-500MHz, and 850-1300MHz. It supports 3 bands: HF & VHF 144MHz, UHF 430MHz, and 1200MHz.
This device combines the antenna inputs of the above 3 bands into one using this triplexer. I think it's very convenient.
